Your CARES Package can provide someone living with HIV with personalized food for home delivery, transportation, household items, and a cell phone to stay connected. Can also provide someone living with HIV who was recently experiencing homelessness with household essentials.
Prevention Education CARES Packages
Your CARES Package can provide those at risk in our community with confidential HIV and Hep-C testing with the ability to get into care within 24 hours of a positive diagnosis.
Counseling & Community CARES Package
Your CARES Package can provide someone with counseling to address trauma, mental health, and substance use or can provide someone living with HIV educational programs and social groups that help them overcome stigma and isolation.
Comprehensive Health CARES Packages
Your CARES Package can provide medication adherence tools, transportation and may provide a comprehensive first visit to our Nashville CARES Clinic at My House.
