Corrections Navigation
Nashville CARES works with Department of Corrections in Middle and East Tennessee to provide pre-release counseling and linkage to care and services for people living with HIV. Intensive support through case management services are provided once an individual is released to ensure barriers to accessing HIV medical care, and obtaining stability are identified and addressed.
Correction Navigation services are also provided for individuals being released from local, county and city facilities. Additionally, advocacy to ensure access to HIV medications and medical care while incarcerated is provided.
