Transportation & Food

Transportation
Nashville CARES provides several transportation resources to help clients get to medical and social services appointments. The services include:

MTA Bus Passes
In Davidson County, clients without cars can get help with Metro Transport Authority (MTA) services. These include the MTA Special Service ID, Freedom Pass, 20 Punch Pass or Single Trip Pass. MTA ID’s are issued annually, Freedom and 20 Punch Passes are picked up monthly at the HEARTLine and Single Trip passes are issued as determined by need through the HEARTLine.

Gas Cards
The gas card provides a set amount of weekly / monthly financial assistance to pay for gasoline at participating stations. There are a limited number of cards available, so referral to get a card is done through your case manager. Once on the program, cards are mailed to clients and automatically coded each month.

Nutrition
Nashville CARES provides a variety of programs for clients who need help meeting their nutritional needs.

Food Pantry
All clients can receive a set of food bags each month that includes staple food items such as canned vegetables, dry cereal, rice, pasta and other food items. An HIV nutritionist has selected these specific food items to help people living with HIV/AIDS. Bags may be picked up at the HEARTLine or they can be delivered to your home if needed.

Home Delivered Meals
The CARE-A-Van Cuisine program is designed to provide prepared meals to clients in their homes. A box of 5 frozen meals is delivered weekly by volunteers. The box includes the meals as well as dry milk, juices, breads and desserts. This program is intended to help clients who may not be able to cook meals or who may be recuperating from an illness.
 
Liquid Nutritional Supplements
CARES provides Ensure to clients who need to supplement their diet. Supplements must be ordered by a physician or a dietician. The supplements may be picked up at the HEARTLine or delivered to your home.
