CARE Teams provide practical services for clients who need a little extra help.

Tasks such as simple house work, yard work and shopping assistance are just a few of the services CARE Teams can provide. Teams are available on a one-time basis or a Team can be assigned long-term. It all depends on your needs.

To access CARE Teams services, talk to your Case Manager. If CARE Teams support seems appropriate, your Case Manager will write a referral for the program.
