These include: Comprehensive Care Center, Meharry, VA Nashville, Stones River Medical Consultants, Robertson County Health Department, Urban Housing Solutions, Campus for Human Development, both State and local jails and prison and several other community agencies. CARES case managers are working through all 17 counties in our service region and can also meet with you in your home, or at a nursing home, hospice or hospital.
