Case Management helps people living with HIV/AIDS find and get involved in needed resources and services to address the health, emotional, financial and personal issues associated with HIV/AIDS. Case managers work with each client to write up and put into action a detailed CARE Plan which can include:

CARES uses a system of outwardly deployed case managers, so in addition to being able to get services at CARES offices, they are available at several other locations. These include: Comprehensive Care Center, Meharry, VA Nashville, Stones River Medical Consultants, Robertson County Health Department, Urban Housing Solutions, Campus for Human Development, both State and local jails and prison and several other community agencies. CARES case managers are working through all 17 counties in our service region and can also meet with you in your home, or at a nursing home, hospice or hospital.
