The position requires the successful candidate to possess a BSW/BA in Human Services and experienced in HIV, housing, entitlements, and/or TennCare. As a case manager, the successful candidate will provide services at CARES, in client homes and in coordination with other medical and social services providers. While not required of the position Bilingual –Spanish Speaking abilities are desired. AA/EOE.

TITLE: Case Manager
POSITION: Full Time Regular Exempt
SUPERVISOR: Director of Case Management
CASE MANAGEMENT:
1. Provides full range of client/significant other specific social work/case management activities: Intake, Housing, SSI/SSDI, Food Stamps, Entitlements, TennCare, Referral, CARES and Community Direct Assistance, Advocacy.
2. Develops/implements comprehensive client Case Management (CARE) Plan.
3. Maintains needed documentation of service provided: Client chart, Ryan White Contact, and Needs Assessments
4. Maintains work consistent with Department/Team Quality Assurance Standards.
5. Provides community-based social work services: Advocacy for People Living wit AIDS, Education, Liaison/Brokering, and Referral networking
OUTREACH:
1. With administration, identify and establish contact with key medical and social services providers in the community.
2. Establish and maintain relationship with identified medical and social services providers.
3. Identify and link HIV-positive individuals who are currently not accessing services to HIV/AIDS resources and services in the community.
GENERAL DUTIES:
1. Adheres to Agency, Department and Team policies and procedures.
2. Participates in Agency, Department and Team meetings and events, collection of data/information, and Quality Improvement Monitoring and Review.
3. Performs other duties as designated by supervisor or agency
QUALIFICATIONS:
1. Preferred BSW and one year experience or Bachelor Degree in Human Services and 2-3 years paid experience (HIV/AIDS specific experience preferred).
2. Experience with HIV/AIDS, homelessness, housing, TennCare, and/or entitlements preferred.
3. Licensed driver with valid collision and liability insurance.
